Evidence-based Residential Substance Use Disorder (SUD) Treatment and Veteran-Only Intensive Outpatient (IOP) care—restoring hope, rebuilding lives, and reuniting Veteran families.
Whether you are a Veteran seeking Residential or Outpatient SUD treatment or a VA Community Care (CITC) coordinator submitting a referral, here is everything you need in plain, simple steps.
Under the VA MISSION Act, eligible Veterans can receive VA-authorized Residential SUD and Outpatient (IOP) SUD treatment in the community at no out-of-pocket cost.
Call us at (409) 838-3946 or email info@vetcarerecovery.com. Our team will help determine whether Residential SUD or Outpatient (IOP) care best fits your needs and guide you through requesting your VA referral.
Contact your VA Primary Care Provider, Mental Health clinician, or SUD coordinator and request a Community Care Network (CCN) referral for Substance Use Disorder treatment (eligible when VA care is over 60 miles away, unavailable within 28 days, or in your best medical interest under 38 CFR § 17.4010).
Tell your VA provider to send the referral to Reboot Investments, LLC (dba VetCare Recovery & VetCare IOP) using NPI: 1174381107 or Contract #: 400TW85012T01. You can also hand them our 1-Page Printable VA Referral Sheet (PDF).
Our contract for both Residential SUD beds (in partnership with Land Recovery) and Outpatient / IOP SUD care is held under Reboot Investments, LLC for fast lookup in HSRM.
